Understanding De Quervain's Tenosynovitis: Commonly Known as "Mum Thumb"

De Quervain's tenosynovitis, often referred to as "mum thumb," is a painful condition that affects the tendons on the side of the wrist at the base of the thumb. It commonly occurs in new mothers or caregivers who repeatedly lift and hold their babies, but it can affect anyone who performs repetitive gripping and twisting motions.

Causes of Mum Thumb/ De Quervain's Tenosynovitis

De Quervain's tenosynovitis develops when the tendons at the base of the thumb become inflamed or irritated and inflammation occurs in the synovial fluid surrounding these tendons. Common causes include:

  1. Repetitive Movements: Activities such as lifting infants, carrying heavy bags, or performing tasks that involve gripping and twisting motions of the wrist.
Overuse from activities such as golf, carpentry, fishing, piano and office work.

  2. Strain from New Parenthood: New mothers may experience increased strain on the thumb tendons due to repetitive lifting and holding of their babies.

  3. Inflammatory Conditions: Conditions such as arthritis or inflammatory diseases can increase the risk of developing tenosynovitis.

  4. Anatomical Factors: Certain anatomical variations or injuries to the wrist may predispose individuals to De Quervain's tenosynovitis.

Symptoms of De Quervain's Tenosynovitis

Recognising the symptoms of De Quervain's tenosynovitis can aid in early diagnosis and prompt treatment. Common signs include:

  • Pain: Pain and tenderness at the base of the thumb, which may radiate up the forearm. Pain gets worse when moving the thumb away from the fingers, grasping the hand or moving the pinky finger towards the arm.

  • Swelling: Noticeable swelling or thickening around the thumb and wrist.

  • Difficulty Gripping: Difficulty with activities that involve pinching or grasping, such as holding a baby, turning a doorknob, or gripping objects.

At Tugun Osteopathy osteopathic treatments focus on reducing inflammation, restoring mobility, and promoting healing of the affected tendons. Treatment for thumb pain includes:

  1. Manual Therapy: Gentle manipulation and soft tissue techniques help reduce inflammation and improve the function of the affected tendons. Articulation techniques to the wrist and arm help to restore mobility and soft tissue release helps to decrease inflammation and muscular tension.

  2. Splinting: Temporary splinting or bracing of the thumb and wrist may be recommended to rest the tendons and promote healing. We recommend this brace, or something similar.

  3. Stretching and Strengthening Exercises: Personalised exercises and stretches are prescribed to improve mobility and strengthen the muscles around the thumb and wrist. We will teach you the best exercises for thumb pain relief.

  4. Postural Education: Guidance on ergonomic practices and modifications to daily activities help prevent aggravation of De Quervain's tenosynovitis.

  5. Advice and tips on how to appropriately lift your baby to prevent aggravation and overuse of these tendons.
